Patient Pathway Plus – Cancer Pathway Management
For full visibility of all cancer pathways in real time
Transform cancer pathway management by looking forwards not backwards, advancing patients continuously along their intended pathways. The key is to make the full pathway visible right up to the day of treatment, powered by automated and unified data. This unlocks dynamic escalation and shorter waits. Staff spend less time obtaining data and producing reports, and more time improving pathway conformance, and solving the root causes of delays and bottlenecks. The result? Faster care and improved patient outcomes.
Patient Pathway Plus – Cancer Pathway Management (PP+ CPM)
- Full visibility of cancer pathways for active pathway management
- Collaborative real-time operational tool, reducing duplication of effort for cancer teams wherever they are
- Unified data for automated, accurate national and local reporting
- Full and auditable patient event tracking data for dynamic escalation, to spot bottlenecks, breaches, and long waits and enable timely interventions for effective cancer care
- Increased productivity, immediately saving hundreds of quantifiable hours in manual and ad hoc reporting
- Single view of operational data for evidenced service improvements
- Tracks patients in real time between care settings and hub/spoke services
- Streamlined data for secondary uses such as CPD, clinical audit and research
- Track record of rapid implementation
Continuous waiting times reporting
PP+ CPM unifies all the necessary administrative, diagnostic, and treatment data, compiling pathways and automating national reporting and local business intelligence.
PP+ CPM provides continuous waiting times visibility, with up-to- date, configurable reports that incorporate Cancer Waiting Times rules directly into the system.
Data management
PP+ CPM automatically acquires and standardises disparate data to create a Unified Data Layer (UDL®). Sources include Electronic Patient Records (EPR), pathology and radiology, with real time HL7 updates available.
The UDL is consistent across every implementation, with data and reporting continuity if source systems are upgraded or replaced.
Supporting waiting times targets
PP+ CPM provides dynamic generation of patient cohorts. This powerful sorting and filtering means trackers can quickly identify patients who need action.
It makes all actions visible in real time to avoid duplication of effort, and provides both workflow management and a complete audit trail of work in progress.
PP+ CPM generates cohorts from multiple data points, for instance to identify patients who are at risk of breaching, or missing intermediate pathway milestones.
It is highly configurable. This allows it to conform to national and local requirements, and deliver to every user and team the contextualised information that their role requires.
Visualisation of pathways for informed decisions
Full visualisation of patient pathways ensures complete and contextualised data at the point of care and MDTs, reduces duplication of effort in local decision-making, and promotes integrated care across a geography for better patient outcomes.
